Pioneering Care Partnership (PCP) can help people living in Newton Aycliffe or Darlington who have been recently diagnosed with Type 2 Diabetes and those told that they are at risk of developing the condition.
The project aims to improve people’s knowledge, skills and confidence; enabling them to take control of their own condition by promoting positive health and wellbeing.
PCP Health Trainer Donna Kelly said: “We encourage positive behaviour changes and provide participants with the information and resources that they require to make changes and hopefully improve their health and wellbeing. The programme will entail attending information sessions and one-to-one appointments to review goals. We also welcome carers and family members to attend the sessions as well.”
